Here's a tip whenever you'll be asking someone to transact for you on your behalf. So that your representative will not go home empty handed or depressed because he lack some standard requirements. Authorization letter is a SOP in most government transactions.
1. Make an authorization letter it could be handwritten on a clean sheet of paper or much better computerized.
2. It could go like this :
Sample authorization letter:

To Whom It may Concern
Good day, I (Your Complete Name/ Firstname, Middle Name or Middle Initial, Last Name) authorized Mr/Mrs/Ms. (Complete Name of your representative Firstname, Middle Name or Middle Initial, Last Name) (state your relationship to your representative) to (Clearly state your purpose - Whether it be get your sss number, philhealth no. pagibig number, register him/her on your behalf etc) on my behalf (and state your reason why you can't transact physically in that said government office)

4. If possible also bring the physical IDs/Documents for verification just to be sure.
